Dr. Gregg A. Miller graduated in 1993 from the Sophie Davis School of Biomedical Education at the City University of New York with a degree in biomedical sciences. In 1995, he earned his medical degree from SUNY Health Science Center at Syracuse. He completed his residency in internal medicine in 1998 and thereafter, a fellowship in Nephrology in 2000 at Columbia Presbyterian Medical Center. He sought further training and completed a fellowship in Critical Care at St. Vincent’s Hospital and Medical Center in 2001. Due to his interest in improving the standard of care for outpatient dialysis vascular access services, he underwent international training from 2002–2003 with Dr. Israel Schur, a published expert in dialysis vascular access care.
In 2010, Dr. Miller was appointed Adjunct Assistant Professor at The Sophie Davis School of Biomedical Education in the Department of Physiology and Pharmacology. Currently, Dr. Miller is board certified in Internal Medicine, Critical Care Medicine, and Nephrology with an additional certificate in Interventional Nephrology.
Dr. Miller has emerged as a leader in dialysis vascular access care and is quickly becoming recognized for his work. With the dedication to the goals of the CMS Fistula First program, his expertise is focused on fistula thrombectomy procedures and fistula maturation. In addition, he has developed a working relationship with a network of Vascular Surgeons who specialize in fistulae creation and share the same goals in dialysis access care. Dr. Miller has lectured on dialysis access care and fistula maturation for the American Nephrology Nurses Association and at several ESRD Network meetings.
A dedicated life work and experience with more than 13,000 dialysis related procedures has led to creation of the MILLER procedure (Kidney Int. 2006 Aug;70(4):765-70. Epub 2006 Jun 28). Dialysis Associated Steal Syndrome (DASS) can now be easily, safely, and quickly repaired using the technique first described by Dr. Miller and American Access Care physicians. The Minimally Invasive Limited Ligation Endoluminal-assisted Revision can be applied to patients with a cold, painful, numb extremity (Steal Syndrome) to restore balance in circulation. The MILLER procedure has revolutionized the treatment for DASS and is now being applied to patients who require flow reduction. In most cases the patients have immediate improvement in symptoms avoiding other invasive surgical procedures.
Dr. Miller’s work has been published in the American Journal of Kidney Diseases and recently featured in Endovascular Today. He is a member of the American Medical Association the New York Society of Nephrology, the American Society of Nephrology, and the American Society of Diagnostic and Interventional Nephrology.
Salvatore J.A. Sclafani, MD

Graduating from SUNY Upstate in 1972 and completing a residency at SUNY Downstate in 1976, Dr. Sclafani is a Diplomate of the American College of Radiology with an Added Qualifications in Interventional Radiology. He has been a practicing interventional radiologist for the past 40 years. He spent the last decade as Professor and Chairman of Radiology and Professor of Surgery and Emergency Medicine at the Downstate Medical School.
He is a past President of the American Society of Emergency Radiology, a Fellow of SIR, and the first international member of the Chinese Society of Interventional Radiology. He is also a Founding Member of the International Society for Neurovascular Disease. Recognized for developing endovascular techniques of hemorrhage control, Dr. Sclafani was honored by the New York Academy of Medicine for his work among the poor and by the New York Police Department for his celebrated save of a police officer wounded in a shooting. Dr. Sclafani has also been recognized for outstanding community service by Concerned Women of Brooklyn.
Dr. Sclafani has authored more than 160 scientific papers and authored or co-authored 20 textbooks. He has also presented more than 250 invited national and international lectures. He was the first Section Editor of Radiology for the Journal of Trauma, and served on the editorial boards of the Journal of Interventional Radiology, and Emergency Radiology. Dr. Sclafani has been named “Top Doctor – New York City” in Vascular & Interventional Radiology by Castle Connolly in 2005, 2006, 2007, 2009, 2010 and 2011. Castle Connolly identifies Top Doctors by location and specialty using a peer nomination process.
